Answer: After your phone interview, we will send you a packet of information including forms that need to be filled out and returned to us. You will need to see your doctor in order to get medical clearance before coming into our day treatment facility. Our office staff will be in contact with you while you are on our waiting list. You can also request a free assessment or consultation class.
